Failure Case Studies

Practical engineering examples examined to understand what failed, why it failed, which assumptions mattered, what evidence revealed the mechanism and what engineers can learn from it. This is the application and example layer — not the canonical technical reference for individual failure mechanisms such as fatigue, fracture, buckling, joints, composites, thermal or pressure failure, which have their own dedicated Knowledge domains. Cases are organised by failure mechanism: fatigue and crack growth, fracture and brittle failure, buckling and instability, dynamic and vibration, joint and fastener, weld, material and manufacturing, composite, thermal and environmental, pressure and containment, load path and structural detail, modelling and analysis, test and validation, and systemic multi-factor failures. Each case connects structure, load path, mechanism, evidence and lesson to the wider Knowledge Base.

Seismic

Seismic Anchorage Failure: Insufficient Anchor CapacityA case study examining the failure of equipment anchorage due to insufficient anchor capacity under seismic loading, with analysis of root cause and lessons learned.Cabinet Overturning: Inadequate Anchorage and High CGA case study of cabinet overturning due to a high centre of gravity combined with inadequate anchorage, illustrating the importance of CG height in seismic stability.Support Structure Failure Under Seismic LoadingA case study of a support structure failure under seismic loading, examining the failure of a welded connection and the role of stress concentration.Insufficient Bracing Under Seismic LoadingA case study of a structure that failed due to insufficient bracing under seismic loading, examining the role of bracing stiffness and load path continuity.Resonance Amplification from Equipment-Floor Frequency MatchingA case study of resonance amplification that occurred when equipment natural frequency coincided with a floor response spectrum peak, causing excessive response and functional failure.Equipment Interaction: Cabinet-to-Cabinet Dynamic CouplingA case study of dynamic coupling between adjacent cabinets that altered the seismic response of both, causing unexpected anchor loads and internal component damage.Incorrect Mass Representation Leading to Non-Conservative Seismic ResponseA case study showing how incorrect mass representation — omitting non-structural mass and lumping equipment mass incorrectly — produced non-conservative seismic results.Shake-Table Fixture Amplification During Qualification TestingA case study of fixture amplification during shake-table testing, where the test fixture introduced dynamic amplification that distorted the test input and produced misleading qualification results.
